This is an unlicensed 5 player adaptor from Performance Accessories (a division of Interact Accessories/STD), released not long after the first multiplayer adaptor, around 1993-1994. In fact, it is the only unlicensed 5 player adaptor I believe. It functions just like its licensed counterparts, the SuperLink and Super MultiTap, and allows you to play games that support 3 or more players. It is half circle shaped, 6 3/8" long at its straight end, and 1" thick. It had the four ports on the circular end, each labeled 2-5, with the numbers in a weird font, like the one used in the "Babylon 5" TV show logo. There is also a switch on top, like on the other 5 player adaptors, to switch between 1-2 and 3-5 player mode, and the unit plugs into the player 2 port on the SNES deck.

This device may have also been sold in Europe as well as North America, there is a European recycle logo thingee on the back, but can't be sure (it would work with a Euro SNES, of course, as well as a SFC).

It should be noted, since this is an unlicensed accessory, and does not carry Nintendo's Seal of Approval nor the Multiplayer Adaptor compatibility seal, there may be minor incompatibilities with games that would use the standard (licensed) 5 player adaptor system. It is unlikely, however possible, so keep that in mind if you purchase this adaptor. Of course, this adaptor can be had for cheap now, I got mine for $2.99 at a local EB.
